Scott, how long would you give desiprimine before

So I thought I would give this a second go but then I started to go through my posts, etc. and saved email from my pdoc. I gave up on it like a little over two months ago.

Was three weeks enough to know if it was gonna do the job? It is much more stimulating than Nort. and this causes anxiety and loss of sleep, etc.

I only got up to 20mg. last time and my doc was wanting me to go up to 50.

So basically I'm second guessing my decision to try it again.

What do you think? It really is different for me than Nort.

There's always so much to consider when choosing a med. I'm on day three of desiprimine and I'm starting on 5mg. instead of 10mg. I don't think you need a lot when your taking it with paxil. It seems to me like Dr. Gillman said I would only need like 20mg. I'd have to talk to him again to be sure. I sure like him though.

Anyway, I would love this to work. That's why I'm starting out slow. Last time I think I moved up to quickly.

Hi.

For people with mild to moderate depression, 150mg is about the right dosage. For more severe depressions, 200-300mg is often needed. If you are at the right dosage, you might feel some improvement within 14 days. At 20mg, I doubt you would respond. Anxiety very often is a startup side effect of desipramine. So is a rapid heart rate and palpitations. If it is the right drug for you, the anxiety will likely disappear.

These dosages are just guidelines. Some people are rapid-metabolizers of tricyclics, for whom higher dosages are necessary to produce a therapeutic effect.

You need to be on a theraputic dose for atleast 4 weeks to see if its going to work.

20mg per day is an extremely low dose, and i think you'll need to make an effort to go higher, maye to 75mg per day?

My question now is does paxil potentiate (I hope this is the right word) it like it does when taking Nort.? The first day I took 10mg. and yesterday and today 5mg. and I have at times some pretty serious anxiety.

I'm going to try to talk to Dr. Gillman soon to find out what he thinks.

For me, I find that I need the same full therapeutic dosages of drugs when they are used in combination as when they are used by themselves, provided they are of different mechanisms.

The only thing I'd say is that paroxetine inhibits the metabloism of tricyclics, so you might need a dose that is lower than average (maybe 75mg per day instead of 150mg per day?)

It might be better to change from paroxetine to sertraline to avoid the drug interaction

Oops.

Yes. Good catch!

Reducing the dosage of TCA by 50% when combining it with Paxil or Prozac is appropriate. Taking Zoloft instead of Paxil would obviate the need to adjust the dosage of TCA. However, it is good to know that Paxil can be used if necessary.
