For people with mild to moderate depression, 150mg is about the right dosage. For more severe depressions, 200-300mg is often needed. If you are at the right dosage, you might feel some improvement within 14 days. At 20mg, I doubt you would respond. Anxiety very often is a startup side effect of desipramine. So is a rapid heart rate and palpitations. If it is the right drug for you, the anxiety will likely disappear.
These dosages are just guidelines. Some people are rapid-metabolizers of tricyclics, for whom higher dosages are necessary to produce a therapeutic effect.
