HELP!! In 3 weeks my year will be over for my participation in a study group for Agomelatine. This has been the best medication to actually treat my severe depressive disorder. One year ago I was truly ready to give up. I had had enough. I participated under duress in one last clinical trial and l must say it saved my life. Agomelatine has been the only medication that has worked for me. Now my year is ending in a few weeks and I have been informed I can no longer receive this medication until FDA approval!! What am I supposed to do?? No SSRI has ever been effective for me and I cannot believe that my succes on Agomelitine will be suddenly stopped. I am scared to death (literally). Can anyone help me find where I can go to continue my medication regimine? Is it available anywhere?? HELP

I can understand your worry. It's very frightening to have the only thing that has worked for you taken away. I am in this position right now too.

You mention SSRI's haven't worked for you. I'm in the same boat. But have you had any fair trials of TCA's or MAOI's? These might be things to consider as they are often effective for people who don't respond to SSRI"s. Maybe if you tell us which meds you have tried, we could make some suggestions about what to do next.

Unfortunately, I doubt you will be able to find agomelatine anywhere. It's still a research agent only, and to my knowledge, it has not been approved in any country.

Beware of any retailer claiming to have agomelatine available, as it will likely be fraudulent.

IMO, your best bet would be to see if you can get a letter from your pdoc asking to continue to supply agomelatine to you on a compassionate basis. I have heard of this VERY RARELY happening before.

Good luck. I too am very excited about the prospects of agomelatine, and it's intresting to hear that you had such a positive response to it.

Can you tell us anything else about your experience? Effects, side effects, etc.) I'm sure others here would be very interested!

Hi. Have you considered trying melatonin? Agolmelatine, I gather, works similarly to melatonin, by activating the same receptors. It also blocks serontonin type-2c receptors, a type of activity that I believe it shares with Serzone, so that is something else you could try. Apart from that, there are many other types of antidepressants other than SSRIs that you could try: older ones like MAOIs and tricyclics, newer ones like Effexor, Strattera, etc. You can learn a lot about various antidepressant drugs from reading this board.

Rest assured, you're far from out of options even if you can't keep getting the experimental medication. I would try that first, though; if you're on something that's working for you, it's best to stay on it if you can. Talk to your pdoc and/or the doctor in charge of the study, explain your problem. Hi Linkadge, I don't get this exactly. Melatonin is a depressogenic while low doses of mirtazapine only acts as a powerful antihistamine. Cyproheptadine (periactin) is also found to be depressogenic. How the combination mimics agomelatine? I have such an enthusiasm on agomelatine, but if what you suggest is the true thing, I am afraid there would be nothing to hope for...

Very interesting the conducters of the study won't allow you to keep on the medication...I would have a strong freind/family member who doesnt take 'no' for an answer to look into that and talk to some people with power into letting you stay on it saying it will be life threatening to get off it....
Sure there are plenty of other options available, but who wants to go through the experiment process again and all the pain and suffering of finding something that works as effectively...Don't take no for an answer (at least I don't; if I dont have the energy or will to get my needs met, I send my girlfriend or stepmom after them. They know how to make waves!)

I certainly sympathize with your situation. I was also in an agomelatine study and did fairly well with it. Although I wouldn't go as far as to reflect that it was the most impactful med that I have taken, it has definitely been the one with the most negligible side effects.

Now that the 6-month study is over, I'm back on my old standby of a low-dose Cymbalta and Nefazodone cocktail. If available in the future though, I would definitely look to use it again within my regimen.

In terms of receiving authorization for "compassionate use" -- to retain access of a drug after the end of a clinical trial -- this is not typically implemented for most antidepressant studies. Such dispensations are usually reserved for end-of-life or orphan drug trials for indications such as cancer, ALS, and the like. Obviously, this is disappointing for many of us who find significant benefit from study drugs.

Unfortunately, agomelatine is not yet made available anywhere on the market yet. However, agomelatine does have a chance of coming to market soon though. In the E.U., it is rumored that the EMEA might rule on Servier's resubmission of its marketing application as soon as this month. If deemed positive, however, it would most likely take another few months for pricing and manufacturing discussions to be finalized before it comes on market in most Euro countries. In the U.S., as you probably know, Novartis will wrap up Phase III trials within the next couple of months. If the data turns out significant, plans would be followed through on to submit to the FDA by end of year. Given the FDA's recent track record, a good guess is that it will take another 9-12 months from the time of submission for an FDA response and market availability.

I hope this at least offers a little hope for things on the horizon.

May I ask what is your dose of cymbalta and serzone as you said you take low doses. Is this combination helpful for sleep and anxiety? Do you take low doses because of med sensitivity (that is my problme).

I don't suffer from any OCD symptology, so I can't necessarily comment on its impact, however, agomelatine did have slight anxiolytic qualities.

As for my Cymbalta/Nefazodone, I have gone anywhere from 5-10 mg of Cymbalta together with 10-50mg of Nefazodone. Currently, I am on a dose of 7.5mg of Cymbalta combined with 10 mg of Nefazodone. As you guessed, I am very med-sensitive. I exhibit atypical features, and most likely would fall within a soft bipolar diagnosis.

The low-dose Nefazodone is a great off-setter to SSRI/SNRI side effects. It helps as a hypnotic, mild antidepressant/anxiolytic and adds even mood-stabilizing properties. It of course has its own side effects, but are typically less averse at lower doses. And despite the concerns of liver toxicity that were covered a few years ago, I still believe that the issue is minor when using lower doses and monitored properly.

A person suffering from treatment-resistant depression who might be looking to explore an add-on AP (e.g., Risperdal, Seroquel) should perhaps trial Nefazodone first.

When the newer class of 5-HT2a antagonist sleep agents reach market, I would consider trading in the Nefazodone for a compound with better selectivity.

Why not try a lawyer, in combination with a pdoc, it can't hurt to try if it's your only hope. I hope you get what you need.

Hi, I too am very interested in agomelatine. I sympathise with your situation and I know what its like to have the 1 drug that works suddenly taken away from you. For me it was citalopram, it and all other SSRI now give me painful gastritis , great! Currently Ive been put on a 50mg a night of the Tricyclic 'Dosulepin'. Three weeks in and its not too bad, little drowsy (nothing compared to 15mg mirtazapine) and tiny bit of dry mouth. Anxiety and mood seem ok, so maybe you should give it a go.
I'm also very glad to hear of someone having a positive experience with Agomelatine. I've been following the progress of this drug for about a year now and it does sound like something pretty special for depressiona and anxiety. I was wondering if you can tell me what its like for sexual side effects? (they claim it has none!).

Agomelatine powder can probably be acquired from some chemical suppliers.

Unfortunately, mirtazapine, nefazodone, and cyproheptadine all block serotonin 5-HT2A rather than 5-HT2C receptors, so they can hardly emulate agomelatine. As far as I know, there is no reasonable choice for for a 5-HT2C antagonist on the official pharmaceutical market.

heh... I read the outline for that 52week study with Agomelatine.. Honestly you really should've paced yourself by taking 1/2 the dose for the first half of the study, then lie/complain it wasn't working well enough then they might have upped your dose to 50mg, so then you'd have double the supply to pace yourself with which could've lasted more than enough to get you through to the approval date if that even occurs. Yea it seems wrong but truth is no one really cares about the discomfort you'll go through when you run out of the Ago as you're about to find out. If you don't fend for yourself no one will, after all they aren't the ones who are going to hurt and actually feel pain when your medication runs out. My guess is that EMEA's CHMP will prob approve Valdoxan this time around (last time it was rejected most likely bc of financial influences from investors for existing blockbuster pharmas like Lundbeck's Lexapro, after all, drugs with unexpired patents can still make money so if you're an investor why bother competing with yourself?) with sluggish FDA approval to follow about a half a year as it has to do its own useless investigation, in my estimate putting Agomelatine's USA release sometime around early 2009 (+/- a few months), if all goes perfectly. You should probably be thankful you were even lucky enough to get into that study. I tried hard to get in but research docs are b-holes and the criteria they must follow makes it almost impossible to get in, they seem to prefer females too. Just one more reason to dislike government bureaucracy and all these USELESS regulatory agencies. It makes me sick to my stomach that ritanserin very similar in functionality to agomelatine could make an afforable and effective add-on drug yet has no chance of ever being sold as a drug. Looks like you will have to wait like the rest of us. On the bright side, agomelatine may never be approved due to market pressure from existing drugs. Therefore, the discomfort you're likely about to feel won't be pointless. I don't mean to sound like a jerk, I can def sympathize AND empathize with what you must be going through right now. You probably just didn't know what to expect going into this study and now it has come to the end of the study and it's cut-off time. One thing that MIGHT help, but prob not too much. If Ago improved your sleep you may be able to substitute Ramelteon (Rozerem) for the time being. It won't have the antidepressant benefit but it may take the edge off anxiety and improve sleep regularity. However, this is def something to discuss with a pdoc rather than take my word. My guess is that going from Ago to Rozerem is likely to be somewhat of a letdown, but prob better than cold-turkey. This almost makes me glad I didn't get into the study as I wouldn't want the possibility of starting Ago, having to stop it, then restarting it up later only to find that it doesn't work the second time around. I really hope this doesn' turn out to be the case for you! Good luck and thanks for your contribution in getting Agomelatine approved for the rest of us! Hopefully that research center helped pay for your gas/parking during your checkup visits.

Let me start by saying hello to everyone i am new here so greetings. I to have just completed a 12 week double blind study for Agomelatine and let me tell you this medication is NOTHING short or a miracle drug. Now saying that i know that all antidepressents react diffrent with diffrent people but i have tried over 20 medications and they have all been total crap!!!! At the end of the study when i was taken off i went through such a low period i didnt even want to live anymore ( It was hell) I to am also fighting doing evreything i can to get back on Agomelatine. My life changed drasticly i was able to sleep finaly for the first time in my life. My circadian rhythm became stable. Got off medical disability got a great job. I was finaly able to cry which i could never do being depressed for years and i became much more confedent and way less overwhelmed by anxitys. Now that i am off slowly i am slipping back to being blinded by the darkness makes me sick to be honest and it isnt fair. I NEED BACK ON IT I WOULD GIVE UP EVERYTHING THAT I HAVE!!!!!

Wow thats certainly a very positive report, and im very happy to read it.
Can you tell us anymore about your experience with the drug? Specifically Im looking for information on side effects. Ive read claims that agomelatine basically has no side effects(except for a bit of dizziness in the first week), is this accurrate?
Also is the claim of no sexual side effects also true?

Wow.. It certainly raise my hope of finding the good med for me when I hear both of you describe this drug as that good compare to all the others you tried !

I feel sorry for you and I imagine what it would feels like to be in your situation. Isn't there any special program in the US where you can ask for a special treatment and receive this med directly from the company until the FDA approves it ?

I think usually the criterion in this circomstance are: 1.Your illness must be potentially fatal. 2. Nothing else worked.

I do think it fits these criterion in the sense that your depression are potentially fatal. You would both have to have a backrecord of failure on meds, which I think you both have, right ?

I think you should post a new thread and ask the people here familliar with this kind of special program.

To answer Negcreep's question about side effects. I had trouble sleeping the first 3 nights that is it!!! Nothing else no sexual side effects no wieght gain no nothing. Infact i lost 6 pounds being on agolmelatine. And noticed my eating patterns getting better probably because i didn't need food for comfort as much anymore. I can't honestly say enough good stuff about this medication. Anyone have any suggestions on how i may get back on this mediction. My docter has tried calling wherever the main place is twice and each time been denied due to the fact it hasn't been released anywhere else in the world. Also anyone know any new info about when this drug is going to be approved in any country??

Thanks for getting back to me on that one. Im really glad that for you the drug has lived up to its hype. Im really looking forward to getting hold of this stuff here in the UK as ive suffered with GAD and depression for 10 years now.
I added some info the the wiki for Agomelatine a few months ago and its still true now.

We're looking at a 2008/9 approval in the UK and Europe (assuming it gets positive opinion). In the US, Novartis should submit a New drug application (NDA) to the FDA by the end of this year. So can be expected in late 2009/10 over the pond. That is of course assuming they let it through. But considering the latest research and these positve reports im hearing , I dont see how they could not.

While I see why to think it could emulate somehow Agomelatine, I really doubt it would in any way feels like Agomelatine. Thank you for the hope! Can anyone tells me how it felt compared to the other antidepressant classes ? Does it feel like an anxiolitic SSRI but without the side effect ? Is one part of the feeling reminiscent of the 'atypical' light antidepressant effect induce by some atypical antipsychotic, lets say Seroquel ? And what about the cognitive side effects (Memory, attention, organisational skills) and motivation ?

Can't believe something that good is comming on the market after all those years! ... don't count on me to wait for the FDA to approve it here in America. The minute it's in the UK -> Hello offshore order !

Yeah I agree with Marty. To be honest Id give it up trying to find combinations of other drugs to approximate the effect of Agomelatine. Its a totally new mechanism of action, and poly-pharmacy will usually just increase unwanted side effects.
Also it seems as though getting hold of this drug before its approved will be next to impossible. Im not saying its not worth trying, but dont get your hopes up. Id say the best bet is think back through everything youve tried so far and find something that helps and has bearable side effects (trying to get by on a lower dose could help too) then just hold out untill this agomelatine is released.

It sucks but we're just gonna have to be patient.

Exactly. and we dont' even start talking about the fact that what we know about the mechanism of action of a drug is very often suspected to be simplistic in theory and more complex in practice; many years from now we could find that Agomelatine act also on another familly of receptor/transporter/second messenger etc etc... You can't emulate a drug that you don't know well enough. Damn... even SSRI between them aren't often interchangeable depending of your condition. If Celexa can't emulate Paxil and Paxil can't emulate Prozac .. good luck emulating Agomelatine with a strong and dull atypical antipsychotic + the only other med that you know interact with Melatonin Receptors beside Agomelatine!

Did I read well about the possibility that Agomelatine approval in Europe could be in September 2008 ? I'm waiting for something like this since 16 years with intensive trial since 10 years... I, for one, can wait another couple months! .. but I know it's not the case for everyone and I feel sorry for them.
