In today's news, we have:
http://news.yahoo.com/s/nm/20060509/hl_nm/grapefruit_dcthe new discovery that furanocoumaridins (sp?) are the guys in grapefruit juice that interfere with the metabolism of some drugs. Ooops, did I say new discovery? Maybe not-here's an article from 2003:
http://ift.confex.com/ift/2003/techprogram/paper_15590.htm
Another thing I like about the new article is that it suggests that the compounds be added to drug formulations so less drug is required. Unfortunately, the furanocoumaridins are poisonous. If you take cyanide with your meds, I'll bet they metabolize more slowly, too. :>} I'm a research scientist, so I'm always astonished when things like this happen...but it is interesting.

As always, the dose makes the poison.
I wonder how it would change if one was to concurrently consume a 3A4 inducer, like St. John's wort, with an inhibitor. Which would win?

In my experience st johns wort would win hands down in a contest opposing grapefruit juice, except maybe if you drank a few gallons a day. SJW metabolism effects are very very strong, in both induction and inhibition, depending on the med it is combined with. Though we usually hear about how sjw lowers the effectiveness of many drugs by increasing their metabolism, it also raises zyprexa levels by 300%. Happened to me. It was scary. It was through a different route than 3A4.

Of course the questions is, who really likes to drink grapefruit juice? *makes sour face* Yuck! I tried to like grapefruit, and I just couldn't. I wonder if the grapefruit and certains meds thing is a problem for anyone in real life? I mean, I guess it had to be, since they found out about the interaction. Hmm. I just don't know anyone personally who really likes grapefruit.

I was interested because I eat a grapefruit every day-I peel it and eat it, so any nasty enzyme-interfering species, I'd assume, are present in as high an amount as they are going to be. I also take Xanax, and have not noticed any interaction whatever-and yet Xanax has a grapefruit warning right in the package insert.

I lived in Florida for 22 of my almost 26 years, and I still couldn't warm up to it. Yes, to each his/her own. I'm glad you like them, because I always wondered who actually eats those things! LOL
