I suffer from depression (19yrs), nightmares and some anxiety. I started treatment a few years ago and have mainly been using Cipramil. It works for a while to a certain extent and then I get bad again. At the moment I'm on 60mg. Each time it works for a shorter and shorter period. A little over a month this time and then I crash further and further. I'm having trouble believing that I'll ever get better and was wondering if anyone had any advice or happy sucess stories about long term depression? I'm sure people have asked this a hundred times before - but I have the concentration of a very small goldfish at the moment and can't quite seem to work up the energy to run a search. Any comments greatly appreciated. I really need to retain some hope that I might be okay some day in order to not give up trying to get better.

I understand your discouragement but I do believe there's hope. I've challenged several psychiatrists with this problem of poop-out. Many, if not most, are clueless. But we can help ourselves. I believe Cipramil is also know as Celexa. It worked for a while for me then diminished. There are options. Many people add Wellbutrin or other meds to Celexa. Eventually I switched to Wellbutrin combined with Elavil. Plus I take many "brain vitamins" such as fish oil. Right now I'm doing better. There are many suggestions on the alternative board about enhancing AD's with nutrients. Remember too that many new meds are coming on-line all the time like the Ensam patch someone mentioned below.

Some doctors use low dose naltrexone for poop-out.
Have you tried MAOI's?
Drugs in developement are geared towards refractory depression; hang in there!
Search for the PB poster jrbecker; he posted a list of what's in the pipeline.

What I believe are the best remedies for SSRI "poop-out":
1) Dopamine agonists (bromocriptine, ropinirole, pramipexole, cabergoline, etc.)
2) buspirone
3) bupropion
4) NMDA antagonist; amantadine is a DA agonist and NMDA antagonist. I do not know if memantine also induces DA release like amantadine.
5) mirtazapineGiven that I have no real clinical experience, none of those are ranked any higher or lower than any other one. You also can just switch ADs, which is what I did when I noticed every SSRI did nothing for me except make me worse in the long run. :)

Perhaps supplementing with a stimulant (such as Ritalin) would be worth a try.

Gosh. You have so many different directions you can move in to further pursue a treatment that will give you a more reliable response.If you are inclined to continue with citalopram, it might be worthwile to try adding either desipramine or nortriptyline. There is convincing evidence that SSRI + TCA works better than SSRI alone. The suggestion given by another poster of using Wellbutrin is a good one.

It appears that the body (brain in particular) adjusts to AD meds. If so, then one would need to get off that med and onto another one. Ask you pdoc about finding more than one that works, and change them out from time to time.
Cymbalta is good. It's by the same manufacturer of Prozac, the 2d generation Prozac. It's both a serotonin reuptake inhibitor and also a norepinephrine reuptake inhibitor (can give you energy, may cause anxiety in some). You may want to try it as an alternative.

Some suggestions I got from a researcher at NIMH:
cytomel: 25 migrograms per day
folic acid: 2 mg per day.I'm also a fan of fish oil. In particular, you want to aim for about 1000 mg per day of the EPA component.
