prozac for OCD - but 5ht2c blockade ???

Posted by linkadge on April 30, 2004, at 13:47:43

How can prozac be a good treatment for OCD when it is a potent antagonist at the 5-ht2c receptor???

I think it is not helping my ocd and am wondering if this is why.

My sister has depression and OCD (as well as me).
Prozac seemed to knock her depression away, but not the OCD...she just added Anafranil 25mg.

I wouldn't worry about various receptor bindings etc. Some drugs which have no justification for treating OCD (as prescribed by the 5HT theory) can wipe it out in some folk.

I don't know what the difference between serotonin 2A and 2C receptors is, but I thought that blocking s-2 receptors lessened anxiety and improved sleep architecture.

This is true. But the therapudic action of these drugs on OCD and panic, seems to be activation of the 2c and 2a receptors respectivly. (don't know if it is true but..)

Blocking serotonin reuptake is a rather blunt action, but I thought that it was agonisation of s-1a (postsynaptic) receptors that caused the increased anxiety. Does agonisation of s-2 receptors cause increased anxiety? Even if it does, antagonisation by fluoxetine would help, not hinder.

I agree with Keith.

5-HT2A & 2C antagonism is generally seen as good things. 5-HT2A antagonism is said to be anxiolytic & improve deep sleep, all the first generation AP's, Most Tricyclic AD's, third generation atypical AP's, Remeron, Serzone & Trazodone have strong 5-HT2A blockade. 5-HT2C blockade may or may not be anxiolytic, could be a second reason for Remeron to increase appetite & can help with SSRI induced anorgasmia.

Cheers,
Panda.
