Not sure what to tell my new pdoc

Posted by Rakken on January 14, 2002, at 15:43:36

I have had one appointment with my new pdoc. I went to him since my last one wouldn't really listen to me and was frustrated with all the failed drugs I'd tried. I tried Provigil, Prozac, Paxil, Remeron, Risperdal, Depakote, Concerta, and Ritalin. I am currently on Adderall for ADD and I take clonidine at night to supposedly help me sleep. I have a lot of anxiety and am very self conscious. The new pdoc said that he wasn't going to solve my problem the last time I visited. So that got me very disappointed and angry with him. Anyways, he kept pushing SSRI's and anti-psychotics on me. I kept refusing and will continue to. I have had too many bad experiences with the ones I have tried and refuse to mess with them again. So I'm thinking that I will tell him I've done some research and heard that beta-blockers and benzodiazepines work well. My therapist tells me that I should be on anti-psychotics and that SSRI's have shown not to work for me. He says that they are the standard treatment and that I didn't respond to the standard. I think I should tell my pdoc that. My therapist says that benzodiazepines would be the thing for my situation but I don't want to say that to my pdoc because I think he'll label me as a drug abuser and raise a red flag. But I don't think I like him or trust him (from the first visit's impression), so I might be willing to just tell him what I think. Any suggestions on what I should do? I have ADD with high anxiety and social anxiety. I have had depression before and I'm 18 years old. Since I'm young I think the benzo suggestion will cause him to suspect I'm a druggie or a risk of some kind. I'm also considering suggesting Inderal.

You could always ask him what he thinks instead of *telling* him you want to be prescribed a benzo: "I've heard that benzodiazepines really help anxiety without many side effects. What do you think of that as a treatment option?" Something like that. If he thinks you're a drug addict for asking that, he's probably benzophobic and he'd never prescribe one anyway.

Hey,

SSRI maybe the "standard" treatment, but that doesn't mean the most effective. SSRIs are the easiest to prescribe with simple dosages and have little risk of OD/legal risk to the psychiatrist. They have never been proven to be more effective than the TCAs or MAOIs .... in fact many studies show they are inferior. Benzos also scare some pdocs because of medical liability .... which is ridiculous because I haven't seen to many patients suing pdoc because of prescribing a benzo to them.

Get a well seasoned clinician .... one who has been practicing for a while .... they often have seen how effective TCAs and benzos are and are comfortable use them.
