Hey,
SSRI maybe the "standard" treatment, but that doesn't mean the most effective. SSRIs are the easiest to prescribe with simple dosages and have little risk of OD/legal risk to the psychiatrist. They have never been proven to be more effective than the TCAs or MAOIs .... in fact many studies show they are inferior. Benzos also scare some pdocs because of medical liability .... which is ridiculous because I haven't seen to many patients suing pdoc because of prescribing a benzo to them.
Get a well seasoned clinician .... one who has been practicing for a while .... they often have seen how effective TCAs and benzos are and are comfortable use them.
