My boyfriend began taking serzone last week. He has moderate tourette's and OCD. I think he is taking 100-150 mg of serzone 1x a day (morning)currently. He has found that he has fewer rage attacks, but is noticing that he is more vocal about things that are bothering him now. Where he would have stuffed his feelings at work, he is finding he is expressing his dissatisfaction with people or situations that upset him. He feels that it is good that he isn't "stuffing" his feelings and later going into rages, but he is concerned that he is a little inappropriate in expressing his dissatisfaction with certain people to other coworkers. DOes anyone have experience with serzone and tourette's/OCD/rage? Is it possible that he needs to take an afternoon dose? This problem seems to happen at the end of his work day.
Any ideas would be appreciated. I think he really likes the reduction in rage attacks, and the way it helps him sleep. He is simply concerned about being inappropriate at work when he opens his mouth.
Thanks,
cole

Cole - Although there is some debate, some researchers have shown that there is a subset of people with OCD that react adversely to one of the metabolites of Serzone (meta-chlorophenyl-piperazine or mCPP). mCPP seems to exacerbate OCD symptoms in some people with OCD. Perhaps this could be causing his disinhibition. It would be a good idea to as his doctor about this.
Just a thought - Cam

Hi Cole.
I would like to offer one possibility that has nothing to do with an adverse reaction to Serzone. I do not favor one explanation over another.
Perhaps your boyfriend has also been suffering from a moderate depression. Depression can cause people to feel inhibited and uncomfortable in social situations. There is often a lack of assertiveness and a loss of self-confidence and self-esteem. If Serzone has all of a sudden removed the inhibitory restraints of an unrecognized chronic depression, he might need some time to learn how to manage this rush of new feelings of assertiveness that seem to have come out of nowhere. I have seen this happen to me.

Hmmm. You have both given me some good ideas/ explanations. I'm going to print them off and tell him about them tonight.
He's actually taking some of my old serzone that I had left over, because my doctor explained that serzone can help with the rage problem he's had for quite some time. I've definitely noticed a difference in both a reduction of the rages, and a possible brightening of his mood. He has the name and number of a good neurologist in town that my psychiatrist recommended, and I think he'll give her a call in a week or so. He just wanted to stick his toes in the water with the serzone, because he didn't really like any of the other meds he had tried: haldol, neurotin, and clozapine (I think). I know that letting someone else use your meds isn't a great idea, but my pdoc really felt that serzone would help him, and he has a hard time admitting he has a problem, let alone aggressively seeking out appropriate treatment. I appreciate your input, and I'll post again if we have any more questions. Thanks a lot,
cole

Another thought----I read (in psychopharm tips, I think) that SSRIs can have a disinhibiting effect. Serzone is not an SSRI, but perhaps has a similar effect???

I tried Serzone for anxiety and depression [not for OCD]. It made me very agitated and gave me crying jags all the time, after about 10 or 12 days. I also had very bad insomnia with it. I started 50mg 2x day and gradually went to 250 mg 2x a day. I only was on it 4 weeks. I hope it works better for your boyfriend, or that he finds the best med[s] for his condition. I am on 40 mg Prozac now with very good results. It's all trial and error with everyone.
