My insurance paid 100% of the bill like it always did for everything else. No co-pays. I was limited to the number of visits I could have in a set time period, say 6 months 4 visits. You can't just walk into a pain clinic you need to be refered to one by your dr. Usually you are refered when your dr(s) feel they can no longer help you and you need pain management(narcotics) to help you have a quality of life and pain free existance.
