Hey,
This guy is an absolute a** and probably a criminal. I'd second that motion to report him to his health occupation board, as well as your state's department of insurance. They usually adjudicate insurance matters like this. Also your state's Attorney General may be able to help out. I'd be reporting him to the world.
If by chance this happened in Maryland, I can help you out with this type of stuff.
You were treated wrongly and none of it is your fault. None of it.
