I think psycological fragmention is when someone feels like they are falling apart - like they can't hold it together anymore. In concrete terms, it might mean they feel sick, or inert or they might be crying all the time. It may mean you feel six different things at the same time, or it might be switching quickly between moods. A lot of people use this term to mean they have lost their sense of self - they feel in parts and pieces.

Some people describe a horrible sense of melting - like they are losing more and more of themselves in any kind of solid form. I've felt this myself. It causes great anxiety and complete chaos when you don't know who you are.
