When I say it, it is usually something positive I am saying about a friend who is a lot of fun who aren't afraid to be themselves and have a good time.

I say mentally ill when I mean someone is mentally ill.

But I am sure it has a different meaning to others and is used differently by others, so it is probably not the best thing to say unless you know the person and how it will be perceived by them.
