The hand surgeon I used to work with didn't ever do it. He felt it was too risky. I watched a film of the procedure once, and what struck me was that in cutting through the top of the carpal tunnel (shoot, I forget what part that is), there appeared to be a smear of fatty tissue from a nearby fat pad that blurred the screen. That's exactly why the doc I worked with didn't like it. He said that happens a lot.
Of course, this was more than 10 years ago.
Anatomy hasn't changed, but the technique or tools hopefully have.
