Yes, and/or deficient stomach acid. Your body cannot absorb triglycerides, the way fats come in food. You need stomach acid to hydrolyse the triglyceride to free fatty acids and glycerine. They are absorbed separately, and then your liver reforms triglycerides from there.

Bile salts are emulsifiers for free fatty acids. If they (fatty acids) aren't free, though, bile does no good.
