Well, if you're concerned, you can monitor kidney function, and quit the substance at the first sign of impairment. Normally, people who get permanent kidney deterioration from lithium take high (bipolar I) doses for years on end; these are usually people who really benefit from lithium, so it is a pity having to terminate it.
