I am very, very sorry to hear that you have been having such a difficult time. It sounds like you have been in a lot of pain and frustration.
I have not tried any opioids. I asked my doctor for buprenorphine (Subutex). He said not.
What country do you live in? Is there any possibility that you can take intranasal ketamine? According to what I read, a large percentage of people who were refractory to previous treatments respond robustly to ketamine. Others glean a partial improvement. I was in the minority who don't respond at all to it. A friend of mine who is in remission using intranasal ketamine had been a partial responder to a combination of Wellbutrin, Lamictal, and Abilify. Have you ever tried this combination?
One of the things that occurs to me is that you might be bipolar or have an occult bipolar diathesis. Very frequently, ADHD/ADD and bipolar disorder occur comorbidly. Even if you are not bipolar, using lithium as an augmenter to Parnate might help. Combining Parnate and lithium has been used successfully for decades. In the case of unipolar depression, low dosages of lithium (300-600 mg/day) can be sufficient. I find that 300 mg/day is perfect for me. If I go any higher, I experience dysphoria, emotional numbing, and apathy. I don't respond at all to Parnate unless I combine it with either desipramine or nortriptyline.
