Medical errors are America's third leading cause of death, behind heart disease and cancer, according to a study released Tuesday. Errors cause at least 250,000 deaths per year, the report said. "People don't just die from heart attacks and bacteria, they die from system-wide failings and poorly coordinated care," said the study's lead author, Dr. Martin Makary of Johns Hopkins University School of Medicine.
