I think it's conceivable that you might respond to 45 mg of Nardil daily and that your pdoc's decision to basically start you at that dose (after having you take smaller amounts for the first six days, if I'm understanding things correctly) makes some sense, especially considering that you experienced psychosis previously while taking Parnate. From what I've read, Nardil doses ranging from 60 to 90 mg daily seem to be the most likely to produce therapeutic benefits, but responding to 45 mg of the medication daily isn't completely unheard of. I personally like the idea of starting with a dose that's relatively low and then looking at the possibility of trying higher doses later on if you don't respond to what basically seems to be your starting dose. I'd imagine that your pdoc would probably be receptive to the idea of increasing your dose if there seems to be a need for it, so I don't so much see the need to start at a higher dose right away unless there's a really urgent need to do so. Taking a larger dose of Nardil seems like it would likely be something that you could do later on if you feel like more of the medication might suit you well.
So, my view is that what your pdoc prescribed seems to make sense, but that the possibility of taking more Nardil later on is something that you also might want to consider. Others here might have different views, though.
