I think you should hook-up with a doctor who will treat you with high-dosage Parnate outright. If Dr. Stewart will not do this, he might refer you to someone who does. Patrick McGrath, perhaps?
Did Effexor or Pristiq help you more than Cymbalta did? What about trying Fetzima (levomilnacipran) or Brintellix (vortioxetine). I might choose to discontinue Parnate to try these two drugs. I am particularly interested in Brintellix because it is a 5-HT7 receptor antagonist.
Right now, I am having no luck with Latuda or Saphris in treating my bipolar depression. My guess is that the 5-HT7 antagonism makes me feel better initially, to be followed by a worsening due to NE alpha-2a receptor antagonism. Remeron makes me feel worse. It is also a NE alpha-2a receptor antagonist. So... If you feel better with Remeron and you have bipolar disorder or psychotic depression, you might do well with Latuda or Saphris. Did you try Viibryd or Abilify by any chance?
