Have you tried orlistat (Xenical)? I know some people have a problem with it causing diarrhoea (especially if they don't stick to the recommended low fat diet), but it is an approved weight loss treatment and could be worth a shot.

Topiramate (Topamax) tends to cause appetite suppression and weight loss but requires very careful dosage adjustment to minimise cognitive and psychiatric side effects eg. poor concentration. Other side effects such as tingling of the hands/mouth and abnormal taste are also common.

Metformin (Glucophage), the well-known diabetes medication, sometimes produces weight loss (usually small). Various blood tests would be needed first eg. blood chemistry/renal function and serum glucose.
