I have bipolar disorder and did not like the way I felt when I first added Seroquel to imipramine. It made me agitated. Perhaps if I had stuck with it for more than a week, this effect would have dissipated.
My idiosyncratic reaction aside, I see nothing wrong with the combination. I don't think the antidepressant properties of Seroquel have been adequately elucidated, but they do seem to exist. The major metabolite of Seroquel (norquetiapine) acts to inhibit the reuptake of norepineprhine as does nortriptyline. Both Seroquel and nortriptyline block serotonin 5-HT2a receptors, so there is some overlap. Compared to other neuroleptic antipsychotics, Seroquel has a reduced risk of EPS. This might also include tardive dyskinisia (TD). However, we don't yet have people taking this drug for 20 years to confirm this.
