Hey rj,
I tend to agree with Scott regarding multiple exposure to meds and desensitization. It seems to be a good explanation for resistance.
Also, I've seen your posts about struggles with substance abuse. Awhile back, I had a problem too (I'm clean now and have been for years), and I can say (quite confidently) that when using any illegal drugs (or addictive meds/pills in copious amounts), it's impossible to know what's going on psychiatrically, let alone to try and treat it. Even occasional use will tip the scales. I sort of outgrew the addiction, and got to the point where my brain chemistry simply couldn't handle it anymore. But I'm not judging! If you're clean, great, then it's not the drugs; if not, that's okay too, but don't expect to figure things out if you're still using.
Another thought I had when reading your post is the med "food" Deplin. It supposedly helps those with a folate deficiency (because they can't break it down properly), seemingly common amongst depressed people. It is marketed as a way of making psych meds more effective (I believe it helps the brain to naturally produce more serotonin, norepinephrine and dopamine). So, maybe that's an option. I tried it awhile ago, and didn't notice a real difference, but it's possible I didn't give it enough time.
Either way, good luck, and hopefully others here have some more valuable info!
