I used to be self-conscious about people examining what or how I was eating. My mum pointed out that people are far more interested in their own food and conversation! And you know, she was right.
So I doubt anyone, waitstaff or fellow patron, would even notice that you were using your own cutlery unless it had pink polka dot handles or something similarly outlandish.
Perhaps using a large serviette/napkin instead of a bib would make you feel more comfortable? Tucked into your shirt it would function much the same as a bib.

PS: I had a mild-moderate tremor from meds and was pleasantly surprised to find that an individual Pilates program prescribed by my physiotherapist significantly improved things. I was expecting my knee problem to get better but hadn't even considered the tremor! I think it was more than just greater muscle control, that it trained my brain as well. I wonder if there are any hand exercises that might help you? Forum member Christ Empowered has also had some success with supplements. I think Vitamin E was one of them.
