I'm a long term user of Nardil (14 years - on and off - mostly on). The methylphenidate as an augmenter sounds like a good idea, though I have never found anyone to prescribe it. I live in a rural community of 50,000 or so and the expertise in MAOIs is a big ZERO.
I am a large male and I have been on Nardil up to 120mg. The lowest dose that works for my major depression is 60mg. At this dosage the side effects are minimal. I still have some delay in orgasm, but I am 54 years old. Even at 60mg it takes a month or two for the anorgasmia to go away.
The afternoon tiredness is a side effect that used to just knock me out every afternoon. That has went away over time. I still have some fatigue and motivation problems, so I would love to try a small dose of methylphenidate.
I think insomnia is one of the side effects that will have to have ongoing treatment while on MAOIs. I'm lucky in that 50mg of Benadryl(diphenhydramine hydrochloride) and .5mg of clonazepam knocks me out for the night. I'm very sensitive to the medications that affect the histamine receptors.
