If you were schizophrenic, you wouldn't even know it. The hallucinations would seem real to you and you would not probably be aware that anything was wrong. That's why schizophrenics often stop taking their meds. So do people in intense bipolar manias with delusions. I knew a guy who was convinced that people were trying to poison him with meds.
