Update:
Now had the Nortriptyline reduced from 50 mg to 20 mg each morning due to palpatations and feeling more depressed than ever. Seems to work for neuropathic pain at 20-25 mg but it's useless for depression/anxiety/insomnia.
Back on the Trazodone now at 100 mg per night, great for getting a good sleep but not a high enough dose to keep the depression/anxiety under control. Can't be increased due to known interaction between Nortriptyline-Trazodone and risk of serotonin toxicity.
So back to the drawing board (again)...
