Welcome, Joseph Smith! You've provided a great deal of material & I wouldn't presume to try to sort out a diagnosis -- that's a doctor's job -- but I can tell you that over the course of my life I've had many of your same symptoms & that at age 59 I've finally figured out that one of my central problems is ADHD. Ritalin has made a big difference in my quality of life, so I understand where you're coming from.
First, if you are going to treat your ADHD you need to do it consistently, with medication every day, and you need to find the proper dose. That means finding a doctor or psych nurse practitioner you trust and who trusts you. Doesn't your university have a clinic that's open to students? If so, print out what you posted here & take it with you when you talk to the doctor. Also, there are inventories / questionnaires you can fill out that will help make your diagnosis specific.
Finally, it may be true that ADHD is at the center of your problems -- but it sounds like you have social anxiety as well (in my very amateur reading of your post), so in working with a psych doc or nurse or therapist, you might also want to think about medication that specifically targets that issue.
The main thing I want to suggest to you is that dealing with problems like this is an ongoing process & it won't be suddenly fixed with a prescription, even though you feel so much better with the drug. That's why I want to strongly encourage you to go to your campus health center or find a doctor you can work with to sort this out. And please stick around Psychobable -- there are a lot of smart people around here who have been through what you're dealing with.
