clozaril for Rx-resistant severe MDD

Does anyone have experience with clozaril for severe treatment-resistant depression? This a new patient who has tried everything from ketamine at an NIH study through all the tricyclics, MAOIs, SSRIs, atypical anti-depressants, lithium, atypical antipsychotics. Also a fairly long trial of ECT.
Our psychopharmacologist really feels that clozaril would be helpful. However, it's one of the biggest black boxes I have ever seen. There are a lot of logistical hassles regarding the pharma y's being able to dispense it, the labs each week, and the scary potential side effects. It's a lot for a euthymic person much less someone so severely depressed. However, if it were to work. . . .
