I see why you want to go the stimulant route. Actually, if it helped you feel mellow and concentrate, do you think there's any indication that you actually do have ADD? I would never advocate inventing symptoms, but if you do actually have ADD symptoms, then your new doc will need to know.
I do think you need to be careful how you bring up wanting a stimulant with a new pdoc. You'll be giving a new pdoc the list of things you've tried, so that will get the conversation started. Also, I've seen a number of anecdotal reports (can't remember where exactly) where people have found Provigil to be hugely helpful for depression. So that might be another option.
