Have you been through the gamut of conventional antidepressants (other than MAOIs), e.g. TCAs, SSRIs, SNRIs, NRIs ?
The combination of symptoms I admit is hard to treat with one agent -- because the ADD spectrum one would use something like Strattera or even Ritalin and associated agents. It might also help with the depression. But it could worsen the panic attacks.
So polypharmacy is probably going to be an answer -- maybe something like Strattera and Neurontin.
Zyprexa is an atypical antipsychotic and has little to do with Neurontin, which is an AED and acts on GABA by being a derivative of GABA.Zyprexa acts on blocking various dopamine subtransmitters as well as putting a blockade on some serotonin ones.
Adding a small amount of Zyprexa to an antidepressant and the Neurontin could also be a choice, it might be a mood brightener and help the bipolar attributes, but weight of course is an issue, dose dependent typically.
Or you might try Lamictal which could help with the bipolar and while monitoring for serious rash which is its biggest rare but possible side effect, it doesn't have a lot of other major ones and works as an antidepressant additive for a fair number of people.
