I'm sorry you have had this experience.
It is common practice to charge for missed appointments but I can understand the outrage at extenuating circumstances.
Psychiatrists and psychologists do bill for phone time sometimes, especially if they believe that urgent calls become non-urgent, because it does displace other patients, or they may have larger fees if they are out of the country
Phone sessions are another way of having a session if one can't make it in to the office and however payment one is using it generally is charged at the session price.
Its usually not used extensively because a face to face session is much better for observation.
