Hi Michael,
Is your "agitated depression" diagnosed as bipolar? Do you swing from being very down to being angry at everything and lashing out at everybody? I know a person like this and she is diagnosed as "agitated bipolar".Anyway, if you tend towards some mania, be careful when Nardil first kicks in. It can cause some hypomania in people that are not bipolar. I suspect it can also precipitate a manic episode in a bipolar person.
I doubt if your 6mg of EMSAM is going to help shorten the time Nardil takes to work. I am surprised your doctor didn't have you wash out the EMSAM for at least a few days before starting Nardil. For me, I had to get my dosage up to between 75 and 90mg to get the 90% MAO inhibition required to treat my depression. After this target dose was achieved, Nardil would generally work within a month-for me.
Be positive. For many people Nardil is the only medication that works. I was on it for over 10 years. Some people have been taking the medication for over 20 years.
When Nardil does work, it can be like flipping a switch. One day nothing, then the next day-relief! It can be an amazing medication for many treatment resistant people, that have tried everything else. I have personally been on 45+ different antidepressants and augmenter combinations. Nothing worked for me like Nardil.
In my opinion, Nardil is still the Gold standard for the treatment of atypical depression and social anxiety. Nardil + clonazepam is an amazing combination for the treatment of social anxiety.
With your agitated states, you may need to augment with a mood stabilizer. Maybe someone else out there knows more about that.
