Fattoush, I have read several postings on your blog and am very sadden by the trouble you find yourself in. It does not sound to me as if you have good medical care. I would encourage you to find a new shock doc and a new psychotherapist. The relapse rate for ECT is 100% if there is no maintenance ECT and/or medication. People who are on MAOIs are 40% less likely to relapse than people on other meds.
You should absolutely be able to talk about suicical thoughts and feelings without fearing hospitalization. It is your therapist's responsibility to keep you safe but unless you have a plan, are really at immediate risk, the therapy relationship is what should keep you safe.
Is suicide an option? Yes. Who can say when someone had suffered as much as they can? The trouble with suicide and depression is that it is hard to know when you have really exhausted all your treatment options.
