As I'm sure you know, the idea behind adding an anti-inflammatory with an opiate is that the pain will be targeted two different ways: one reduces the cause of pain, and one that interferes with our perception of it.
In theory its a good idea, but I'm just not a big advocate of acetominophen in general. It's a liver thing, plus the fact it just doesn't do much for me.
My personal preference for moderate pain management, when one can afford to sort of "check out" for a while is ibuprofen coupled with phenergan (promethazine). For severe pain that needs to be managed at home it's MS contin.
I work in tandem with a bunch of physicians studying sickle cell disease, so I am well versed in pain management strategies. Percocet is the drug of choice for most patients with this disorder. It seems to be really good for chronic ischemic pain (or soft tissue pain as we refer to it). During acute pain crisis, demerol/dilaudid/morphine PCA appears to be the way to go.
For other kinds of pain, there are very very few options.
