I'm of the opinion that most reaction is placebo/nocebo, but that goes for whats published, as well. It is quite odd though. People who are quite condescending to their patients live in awe of 'opinion leaders', and while a case report published is true, what their own patient is saying isn't. It doesn' have the right kind of validation somehow.
