You have nothing to be ashamed of. After reading your post, I said to myself, this is a person who is clearly in distress, and, more importantly, seems to be quite aware of what would be best for you.
When we are in a crisis mode, our ability to express our current state may sound distorted to the practioner?
I know when I am extremely stressed and feeling anxious, my ruminations can get the best of me. When this happens, everything seems to snowball into something much larger than need be.
If I was you, I would copy and paste your post, have your p/doc or GP read it. This may sound like a strange suggestion, but your post does not sound strange in the least.
It may help you feel at ease when you meet with your doc, let the post speak for itself!
If there is a trust issue with drug abuse, assure your doc that you would be willing to report weekly or however often they want in order to show you are responsable.
