It may be worth trying buprenorphine (Subutex) again, but with methylphenidate (Ritalin or Concerta) and perhaps some dextromethorphan or memantine to prevent or diminish tolerance. Stool-softeners or laxatives may be useful if constipation becomes a problem. Nicotine chewing gum may be a successful substitute for smoking.
