What if you have trouble converting folic acid/folate into its 'active' form, which is what the site Phillipa refers to happens to lots of people? Few antidepressants would help depression caused by such a deficiency. I would call this 'treatment resistant depression'. At least some pharmacuetical companies are starting to admit that its not all just drug, drugs, drugs.
I suspect micronutrient deficiencies play a HUGE role in 'treatment resistant depressions'.
Just look at lithium! When being treated with lithium you are effectively taking 'mega doses' of a micronutrient!
Not to mention St. John's Wort, which has pulled me out of depression better than any SSRI ever has!
