Some doctors say to start at a higher dose, to minimize side effects with Remeron. Totally counterintuitive, isn't it? The thing is, with many of these drugs, they're like different drugs at different dosages. Effexor, for instance, is like an SSRI at low doses, then becomes an SNRI at moderate doses, with dopaminergic activity at the highest doses. It's not an additive effect, where more drug gives you more of the drugs effects -- in these cases, more drug gives you a different drug.
With Remeron, most of the sedation happens at the lower doses. Starting at a higher dosage may actually minimize that effect.
So, you'll have to use your own judgment. If I were deciding for myself, having read what I've read, I'd start at the higher dosage.
