Phillipa, it is what my philosophy has been right along, start with the smallest dose and gradually increase the dose. I could never figure out why a doc would continue to raise a dose when the side effects are clearly indicating intolerance for the dose prescribed. Reduction of the dosage is the only way to go, yet many users think they have to go through horrible side effects to get the most out of an AD. This article states that is not the correct methodology in prescribing the most effective dose of an AD. The dosage should be based on patient response rather than recommnded dose as indicated in the PDR or package insert. Those doses are not indicated for all patients.
