I've read of Pizotifen being helpful for Social Anxiety and I'm curious of anyone who's tried it or heard the same? It's an antagonist of 5HT1, 5HT2A and H1 receptors licensed for prevention of migraine headaches. It's also been found helpful in the treatment of mild depression. I wonder if it would be helpful for the treatment of SSRI induced sexual dysfunction (with its 5HT2A antagonism like cyproheptadine)? It may also lower prolactin levels this way since serotonin promotes prolactin release by stimulating 5HT2A receptors.
According to "Essential Psychopharmacology: Neuroscientific basis and practical applications" 5HT2A antagonism promotes dopamine release in the nigrostriatal and mesocortical pathways leading to improved negative symptoms in Schizophrenia (where atypical antipsychotics with 5HT2A antagonist properties are used) and lower incidence of movement disorders. I was much less anxious in the first month of low dose amisulpride treatment (a dopamine helper) and I wonder if pizotifen could give similar results in promoting dopamine release, but with (hopefully) long-term efficiency? I'm skinny so the increased appetite from pizotifen doesn't matter and the H1 blockade would only help my allergies (and perhaps a bit of cognitive impairment). It also helps sleep because of the sedating properties. Now how to convince my cynical GP of its off-label merits?
