They have 2 options. They can submit forms for a particular patient or get blanket permission for a certain med. If it is in an institutional setting they are likely discouraged somehow from using the more expensive med. The weird thing about this is that Wellbutrin is also also on the resricted list because some doctors prescribe it as an anti smoking aid. Horrors!!! Imagine if everyone quit smoking. It would probably save billions a year in medical costs to the system. I'll bet this cost factor is the reason that we are so slow to get some anti depressants approved. Then they would have to cover the expensive patent meds. In the mean time they are just waiting for the patent to run out.
