Doses usually start becoming effective around 100mg for some, yes... it has a very wide range of dosing up to 300mg and sometimes to 400 and 450. For depression, I would say it makes some modest improvements, it wouldn't be my first choice for more complex depression, I would probably use Cymbalta or the NE activating medications. For OCD it is still excellent, regardless that it is now generic. It just didn't get a lot of market share and the company didn't bother (or didnt bother to put a compelling case for NDA extension) to the FDA, so it is now generic. So you get very good OCD and modest depression coverage, cheaper.
