I'd expect a pdoc to be familiar with use of sarafem for PMDD, so I'd include that w/in her area of expertise--whereas an obgyn would necessarily be as expert about use of other pmeds, esp. those that might work better for you.

I still think your pdoc is probably more on top of this particular problem.
