I've heard of dosages in the range of 200-300mg per day being used although you might not be able to get your pdoc to prescribe dosages that high. The drug company blurbs and the PDR are mostly legal disclamers that say "If you prescribe outside this range or get any of these side effects you have been warned so don't try to sue us".
