I couldn't even imagine that dose. I'm struggling now at 600, and working down. I can't think of a better example of how differently people can react to the same drug.
Gabapentin has unusual bioavailability; as the dose increases, absorption percentage decreases, until at 4800 mg/day, uptake is only 27% (compared with 33% at 3600). That means that you get only a slight boost in total uptake from that large dose increase....less than 100 mg of the 1200 mg dose increase. (All figures based on averages from a number of subjects.)
You might get a better efficacy boost by taking it with food. That accounts for a 14% increase in uptake. Or, do both. Increase the dose, and always take it with food.
I'm thinking my dose will end up being in the 300-400 mg/day range.
