Remeron has about a 1/1000 chance of agranulocytosis (low WBC), give or take... but from what I understand this manifests earlier on and you would certainly be aware if you have been taking Remeron for 2 years; one generally gets unexplained fevers or other flu-like symptoms. But your case could be unique by the combination of medications...

Lexapro and Remeron are both active at the P450 2D6 to some extent, there could be increased plasma levels of one or the other.

I think you should contact both doctors and see what the next course of treatment to do. If there's no immediate reason to not discontinue the Remeron (e.g. current severe depression with suicidality), I would suggest discontinuing it for a few weeks or so to see whether the Remeron is at fault or not, or whether it is due to some other organic origin. I don't think the other medications would be causalities but your doctors would know better.
