Clomipramine is said to be the most effective AD, overall, by many. Unfortunately, the fact that it works on so many systems also gives it more side effects. I take nortriptyline 100 mg, which is effective for depression and for ADHD, more effective on the depression than the ADHD. Aside from terrible dry mouth, the s/e from nortriptyline are relatively mild. But it took a long time to titrate up to an effective dosage because the worst side effects (anxiety, agitation) occur early in treatment. The difference between clomipramine and nortriptyline is that clomipramine could be effective for social anxiety as well as depression. Contrary to what many think, it is no more a specifically OCD med than the SSRI's: it simply has efficacy in this disorder comparable to them.

I have social anxiety too, for which I take 1 mg clonazepam daily (more as needed). The advantage of clomipramine is that it might remove the need for anxiolytic augmentation. Effective dose range is somewhere around 150-300 mg.
