Generally bipolars have significant decreases in grey matter both in the frontal cortex, and in the hippocampas. Lithium increases BDNF in the frontal cortex as well as in the hippocampas.
Frontal cortex hypofunction is seen in quite a few mental disorders but especially in bipolar. This is why some bipolars can actually somewhat stabalize with stimulants, because they activate the frontal cortex.
In any respect, repair of the frontal cortex's functioning can reinstitute control. Frontal activation shuts down hyperactive limbic activity, and perhaps leading to more stable moods.
